建设工程施工安全竣工评价及发放安全评价书办事指南

 

 

一、建设工程施工安全竣工评价申请

施工单位办事人员到市安监站1楼办事窗口递交《建设工程施工安全竣工评价及办理安全评价书申请表》（一式两份，内容详见附件），市安监站签具收件章后，一份退回给办事人员，另一份由办事窗口在当天移交至综合室，由综合室移交监督室。

二、召开竣工评价会议

监督室在收到《建设工程施工安全竣工评价及办理安全评价书申请表》后，由监督室在3个工作日内安排召开竣工评价会议并填写会议纪要。

三、建设工程施工安全竣工评价书发放

经竣工评价会议确认已竣工的工程，在召开竣工评价会议5个工作日后由施工单位办事人员凭市安监站签具收件章的《建设工程施工安全竣工评价及办理安全评价书申请表》到市安监站1楼办事窗口领取《建设工程施工安全评价书》。
